We can discuss any questions you have and determine if my clinical abilities are suitable to your needs. I currently accept Highmark Blue Cross/Blue Shield plans.  If I am not a provider for your insurance company, services may be covered in full or in part by your Out-of-Network benefits or EAP plan.

  
Questions for your Insurance Carrier 


Please check your insurance coverage carefully by asking your health insurance representative these questions:


  • Do I have mental health benefits?
  • What are my out-of-network benefits?
  • Do I need an authorization number or pre-certification?
  • What is my deductible and has it been met?
  • How many sessions per year are covered?
  • What is the amount covered or percentage reimbursed per session?
  • Is approval required from my primary care doctor?

For those who choose to use Out-of-Network benefits, receipts will be provided by request to submit to your insurance company for reimbursement.  


Payment Arrangements 


Cash, personal check and credit cards are accepted for payment at the time of session.  

Other payment arrangements, such as monthly billing, are also available.  


Cancellation Policy


Please provide 24 hour notice to avoid a late-cancellation fee.  Thank you.
