Rates and Insurance

I can answer any questions you have and determine if my clinical abilities suit your needs. Please know that I am an out-of-network provider.  However, services may be covered in full or in part by your insurance benefits or EAP plan.

Questions for your Insurance Carrier
Please check your insurance coverage carefully by asking your membership representative these questions:

• Do I have mental health benefits?
• What are my out-of-network benefits?
• Do I need an authorization number or pre-certification?
• What is my deductible and has it been met?
• How many sessions per year are covered?
• What is the amount covered or percentage reimbursed per session?
• Is approval required from my primary care doctor?

Receipts will be provided by request to submit to your insurance company for reimbursement.   

Payment Arrangements
Credit cards (through IVY pay), cash and personal check are accepted for payment at the time of session. 

Cancellation Policy
Clients must provide 24 hours notice to avoid a late-cancellation fee.  Thank you.
